Idaho's CAO-numbered forms apply across the state, so the same petition and supporting forms work whether you file in Ada County or a rural county.
The petition: CAO_D_1-5 or CAO_D_1-6
Form CAO_D_1-5, Petition for Divorce (With Children), and Form CAO_D_1-6, Petition for Divorce (No Children), are Idaho's standard starting documents. Which one you use depends on whether the marriage produced minor children. You file both together with the Family Law Case Information Sheet, Form CAO_FL_1-1, and the Summons with Orders, Form CAO_FL_1-3. Together these open the case and put standard temporary orders in place.
Financial disclosure: the Affidavit Verifying Income and Inventory of Property and Debts
Idaho's Court Assistance Office publishes Form CAO_FL_1-11, Affidavit Verifying Income, and Form CAO_RFLPPi_1-1, Inventory of Property and Debts, as the standard financial disclosure documents for Idaho divorce and family law cases. When a case involves child support, the office also publishes Form CAO_FL_5-1, Petitioner's/Respondent's Mandatory Child Support Disclosures. Each parent completes this form so the court can calculate support under the guidelines.
Forms for cases with children
Idaho has adopted the Uniform Child Custody Jurisdiction and Enforcement Act. There is no standalone, separately numbered CAO-series UCCJEA jurisdictional affidavit apart from the with-children petition. Idaho appears to collect that jurisdictional information within Form CAO_D_1-5 itself rather than issue it as its own document. If your case involves children, check with the Court Assistance Office forms library or the county clerk to confirm whether a separate UCCJEA disclosure form applies.
E-filing and self-help support
Idaho's e-filing system, File & Serve, is part of the statewide iCourt platform. It's available in every county and optional for self-represented litigants under Idaho Rules for Electronic Filing and Service Rule 4(b).
